B-SYNC synNotch CAR-T clears DMG tumors in mice via brain-restricted activation
UCSF's B-SYNC CAR-T uses brevican-triggered synNotch to restrict EphA2/IL13Rα2 targeting to the brain, clearing DMG xenografts in mice and persisting against rechallenge. The approach improves on prior EGFRvIII-dependent designs but remains preclinical. Human trials are slated for 2027.
{"The team tested intravenous and intracerebroventricular delivery of B-SYNC cells in human DMG xenograft mice. Tumor clearance occurred in 100% of ICV-treated animals; IV delivery also reduced tumors when cells crossed the blood-brain barrier. CAR expression remained off in blood and spleen, confirming spatial control. Re-challenge experiments weeks later showed persistent antitumor activity without systemic toxicity.","This builds on the group's prior E-SYNC platform for glioblastoma, which relied on the unstable EGFRvIII antigen present in only ~20% of cases. By switching to ubiquitous brevican as the synNotch trigger, B-SYNC broadens applicability to DMG while addressing infiltration into brainstem tissue that defeats conventional therapies. DMG median survival remains under 12 months with standard care.","No human data exist yet; the work is strictly murine. Next steps include IND-enabling toxicology and a planned UCSF phase 1 trial starting April 2027. Key unknowns are whether brevican-gated activation will scale to human brainstem volumes and whether antigen loss or T-cell exhaustion will emerge in patients.","Funding sources and long-term follow-up beyond 8 weeks in mice are not detailed in the current report. An independent replication in additional DMG models and direct comparison to non-gated CAR-T would strengthen translational claims."}
UCSF: Phase 1 B-SYNC trial will enroll first patient by April 2027 and report objective responses in at least 3 of the initial 6 participants within 6 months of infusion.
