Multicentre RCT Finds iACT4CARERS Online ACT Platform Reduces Dementia Caregiver Anxiety and Depression at Six Months

A 496-person RCT demonstrated that iACT4CARERS, an eight-session online ACT programme with light NHS support, lowered anxiety and depression in dementia caregivers at three and six months versus usual care. The scalable format targets a population often excluded from mental health services due to caregiving constraints. Evidence quality is high for a pragmatic RCT yet requires confirmation of implementation outcomes and cost savings in routine care.

The multicentre parallel RCT randomised participants across 20 NHS sites to eight online sessions plus written or optional phone support from trained workers versus usual care. Sessions targeted psychological flexibility around difficult thoughts while prioritising self-care actions. Follow-up assessments showed sustained separation between arms, confirming both statistical and clinically meaningful change without requiring caregivers to leave home.

Existing literature documents that one-third of dementia caregivers experience anxiety or depression yet face access barriers from time demands and service strain. This trial addresses that gap by testing a low-intensity, mobile-delivered intervention that needs only minimal professional input, contrasting with earlier face-to-face CBT studies that reported higher dropout. Cost-effectiveness modelling referenced by the NIHR HTA Programme suggests scalability advantages over resource-intensive models.

Longer-term questions remain around ethnic minority reach, given collaboration with the Centre for Ethnic Health Research, and whether gains translate into reduced healthcare utilisation or delayed care-home placement. Next steps include planned implementation pilots and a potential international replication trial to test generalisability beyond England.
