Congo Ebola Outbreak Hits 5,290 Cases by Week 13, Contact Tracing at 16% of Expected
Observational surveillance data indicate the fastest recorded Ebola expansion in Congo, driven by under-detection and a vaccine-ineligible strain. Absolute case counts already exceed prior regional outbreaks at the same interval. Sustained community transmission will require contact-tracing rates above 80% of expected exposures to bend the curve.
The outbreak, declared May 15 but likely seeded in Mongbwalu mining town by February, spreads through highly mobile populations displaced by conflict. Community deaths reached 97% of daily totals on August 17 while just 20% of alerts originate from affected villages. Detection covers only 30-40% of cases, far below thresholds for control. Bundibugyo virus produces milder symptoms than Zaire ebolavirus and lacks approved vaccines or therapeutics, reducing care-seeking and enabling silent chains.
Contact-tracing shortfalls are structural. With 60 expected contacts per case, authorities anticipated 99,780 exposures from 1,663 recent cases yet identified only 19,630, yielding an 82% rate among listed contacts but 16% of the true total. New transmission in previously unaffected Bas-Uele province and slow spread in South Kivu illustrate patchy containment. Historical comparisons show this outbreak advancing 18 times faster in cases than the 2018-2020 event by equivalent time points.
Security constraints and distrust limit classic ring-vaccination tactics. International agencies must prioritize community alert systems and safe burial protocols over vaccine-centric strategies unavailable for this strain. Without rapid improvement in case finding, models project continued exponential growth into late 2026.
Next steps require independent field audits of surveillance sensitivity and deployment of experimental monoclonal candidates already in trials for related filoviruses.
Africa CDC: If contact-tracing coverage stays below 50% of expected exposures through October 2026, weekly incident cases will exceed 800 by December.
