STAREE RCT: Atorvastatin Cuts First Major CV Events 30% in Adults Over 70

STAREE provides the first large RCT evidence that statins reduce incident cardiovascular events when started after age 70 in primary prevention. The 30% relative risk reduction supports guideline revision yet leaves disability-free survival and long-term harms incompletely characterized. Evidence quality is high for the primary endpoint but moderate for broader healthy-aging outcomes.

The double-blind, placebo-controlled STAREE trial enrolled community-dwelling Australians aged 70 and older without prior cardiovascular disease. Participants received 40 mg atorvastatin or placebo, with primary endpoints of major cardiovascular events and disability-free survival tracked over a median 4.5 years. More than 3,400 general practitioners participated, reflecting real-world primary care settings.

Relative risk reduction reached 30% for the composite of fatal and nonfatal myocardial infarction, stroke, and revascularization. Absolute event rates fell from 5.8% in placebo to 4.1% in the statin arm, yielding a number needed to treat of approximately 59 over five years. Disability-free survival showed no significant difference, indicating benefits were driven by event prevention rather than broader functional gains.

Prior trials such as PROSPER reported mixed results in older adults, while meta-analyses in JAMA and Lancet had highlighted under-representation of those over 75. STAREE addresses this gap with a large primary-prevention cohort but leaves questions about long-term adherence, myalgia rates, and diabetes incidence unresolved. Updated guidelines from ESC and NHFA are expected to shift thresholds for initiation in this age group.

Next steps require replication in more diverse populations and longer follow-up to assess net benefit on all-cause mortality and quality-adjusted life years. Observational data from registries will be needed to monitor real-world safety signals post-guideline change.
