Phase III INTerpath-001 Trial: Personalized mRNA Vaccine Cuts High-Risk Melanoma Recurrence or Death Risk by 42%

Phase III data demonstrate a 42% reduction in recurrence or death when a personalized mRNA neoantigen vaccine is added to adjuvant pembrolizumab. The approach personalizes immunotherapy but awaits mature survival data and scalable delivery before broad adoption. Evidence remains at the conference-presentation stage.

The trial enrolled patients with resected high-risk melanoma who had received no prior systemic therapy. Investigators randomized participants to pembrolizumab plus intismeran, manufactured from each patient's tumor neoantigens via mRNA, or pembrolizumab monotherapy. Primary endpoint was recurrence-free survival; the 42% risk reduction (95% CI 0.45-0.75) emerged at a median follow-up still maturing, with an even larger 58% benefit among those recurrence-free past six months. No increase in pembrolizumab-related adverse events occurred.

This result builds on earlier phase II data from similar mRNA-4157 plus pembrolizumab combinations and parallels the shift seen when checkpoint inhibitors moved into adjuvant settings. It also highlights unresolved questions around durability, optimal sequencing with other modalities such as BRAF/MEK inhibitors, and whether the vaccine's benefit persists in patients with lower tumor mutational burden. Health-system logistics for rapid personalized manufacturing remain untested at scale.

Next steps include overall survival readout, regulatory submissions, and trials expanding the platform to other solid tumors. Real-world evidence will be required to confirm whether the observed hazard ratios translate into meaningful absolute gains once baseline recurrence rates and access barriers are accounted for.

⚡ Prediction

Melanoma Institute Australia: Overall survival hazard ratio below 0.75 will be reported within 24 months of the 2026 ESMO presentation.
