Moderna's mFlusiva uses synthetic mRNA to encode hemagglutinin proteins from selected strains, enabling a three-month production cycle versus six months for egg- or cell-based inactivated vaccines. Traditional vaccines for 2025-2026 achieved only 36% effectiveness against medically attended influenza according to CDC surveillance, reflecting both antigenic mismatch and waning immunity after four months. The mRNA platform permits rapid strain updates based on late-season surveillance data from both hemispheres and has shown higher neutralizing antibody titers persisting beyond six months in early-phase trials.
Public health experts note that faster manufacturing does not eliminate the core uncertainty of antigenic drift; strain selection still relies on the same global surveillance network. Real-world impact will hinge on uptake. Annual flu vaccination coverage among US adults over 50 hovers near 55-60%, and post-COVID mRNA hesitancy documented in Pew surveys could offset any efficacy gains if 15-20% of prior recipients decline the new product.
Post-approval observational studies using the CDC's Vaccine Safety Datalink and VE network will be required to measure absolute risk reduction against hospitalization, not merely immunogenicity. Head-to-head RCTs against high-dose inactivated vaccines remain absent; such trials are needed before policy recommendations can shift from the current age-based preference for enhanced vaccines.
