Systematic Review of 81 Studies Links Youth Vaping to Respiratory Symptoms and Cigarette Initiation

The review, led by RCPCH authors including pediatric respiratory specialists, assessed observational and cross-sectional data on exposure and outcomes. Absolute prevalence of regular vaping rose sharply in the decade studied, with vapers showing higher rates of respiratory complaints and nicotine dependence than non-vapers. No randomized trials were available, limiting causal inference. Observational designs leave room for confounding by prior smoking or socioeconomic factors.

Contextual patterns from prior UK and US youth surveys align with these respiratory signals, where flavor availability and point-of-sale displays correlate with uptake. The review also flagged emerging signals for oral health, sleep disruption, and prenatal nicotine exposure but rated those as lower-quality. Regulatory proposals center on flavor limits and packaging parity with tobacco to reduce appeal.

Next steps require longitudinal cohorts tracking incident respiratory disease and lung-function trajectories alongside policy evaluations of flavor bans. Without such data, associations cannot yet distinguish vaping-specific effects from shared risk behaviors.
