Miro1 protein accumulation in skeletal muscle linked to insulin resistance in type 2 diabetes

Miro1 elevation contributes to muscle insulin resistance; exercise lowers it and restores sensitivity. The translational study spans cells to patients and identifies a potential drug target. Evidence remains preclinical for direct inhibition and requires larger outcome trials.

Researchers led by John Kirwan examined Miro1 across cell cultures, preclinical models, and a randomized trial of adults with obesity and type 2 diabetes. The protein sits on mitochondrial membranes and disrupts insulin signaling when overexpressed. In the human trial, aerobic exercise over several months reduced muscle Miro1 content while improving insulin sensitivity and mitochondrial function compared with standard care alone.

The work connects basic mitochondrial biology to clinical outcomes, showing that Miro1 removal causally improves glucose uptake in muscle. This offers a mechanistic explanation for why exercise remains effective even after insulin resistance is established. Prior observational studies had noted mitochondrial fragmentation in diabetic muscle but lacked a specific protein target tied to both insulin action and exercise response.

At the Harold Hamm Diabetes Center, Kirwan plans to test whether Miro1 inhibition can complement lifestyle interventions. Remaining questions include whether Miro1 changes occur in gestational diabetes or early prediabetes and how quickly levels rebound after exercise cessation. Future trials will need to measure long-term glycemic control and cardiovascular events rather than surrogate markers alone.

⚡ Prediction

Kirwan: Phase 1 trial of Miro1-targeted compound begins enrollment by 2028 with safety threshold of no grade 3 adverse events in first 20 participants.
