JAMA Network Open Study of 3.8 Million Veterans Links PTSD and Unmet Social Needs to Elevated Cognitive Impairment

A large retrospective study of nearly four million veterans used NLP to detect unmet social needs alongside PTSD and found a synergistic increase in later cognitive impairment and dementia. The work illustrates how AI extraction of narrative EHR data can expand epidemiologic reach but remains observational and requires confirmation through interventional designs.

The investigators extracted both structured diagnostic codes and narrative notes from Veterans Health Administration records spanning multiple years. An NLP pipeline previously validated at UMass Amherst detected mentions of financial strain, housing instability, and food insecurity that conventional ICD coding often misses. Veterans were stratified by PTSD status and presence of one or more social needs, then followed for incident cognitive diagnoses.

Absolute incidence of dementia was 4.8% among veterans with neither exposure, 7.9% with PTSD alone, 6.4% with social needs alone, and 11.2% when both were present. Adjusted hazard ratios reached 1.92 (95% CI 1.84-2.01) for the combined exposure group relative to unexposed veterans, exceeding additive expectations and suggesting synergy.

Prior observational work has established PTSD as a dementia risk factor, yet most studies relied solely on claims data and omitted social determinants. This analysis demonstrates that narrative-derived social-need signals produce associations consistent with structured data, supporting broader incorporation of free-text methods in large-scale epidemiology while underscoring the limits of observational design for causal inference.

The next required studies are prospective cohorts with repeated cognitive testing and randomized trials of integrated PTSD treatment plus social-service navigation to determine whether addressing both domains alters cognitive trajectories.

VA Office of Mental Health: Within 24 months at least three regional VA networks will report pilot data on combined PTSD-social needs screening linked to cognitive assessments.
