GLP-1 Receptor Agonists Tied to Earlier Sequential NAION in 22,000-Patient Database
A large retrospective database study presented at AAO 2026 found GLP-1 receptor agonist exposure associated with earlier sequential NAION (RR 2.63), consistent with two prior observational reports. The finding is limited by confounding by indication and lack of mechanistic data; prospective studies with active comparators are needed.
Next steps require a prospective cohort with serial OCT and visual-field testing plus active comparator arms (SGLT2 inhibitors) to test whether the association persists after balancing HbA1c and duration of diabetes. Regulatory bodies have not yet issued warnings, but clinicians may consider earlier ophthalmology referral for new GLP-1 starters reporting visual symptoms.
Tin et al.: Within 24 months a peer-reviewed version of this AAO abstract will report hazard ratios attenuated by at least 30% after HbA1c matching.
