Prenatal DEHP Exposure Linked to ASD and ADHD Symptoms via Epigenetic Changes in Australian Cohort
Observational data from 847 pregnancies link third-trimester DEHP exposure to modest increases in early ASD- and ADHD-related behaviors partly mediated by cord-blood methylation. The design cannot establish causation and leaves residual confounding unaddressed. Replication with repeated measures and clinical endpoints is needed.
The Med journal analysis measured DEHP metabolites in third-trimester urine and performed EPIC-array methylation profiling on 800,000 CpG sites in cord blood. Causal mediation modeling showed that a methylation profile score explained part of the association between DEHP and parent-reported hyperactivity, inattention, and peer problems. Absolute symptom increases were modest; children in the highest exposure decile scored roughly 0.3–0.5 SD higher on behavioral scales.
Earlier phthalate studies reported similar associations but lacked mechanistic data or used smaller samples. This work adds network-level epigenetic mapping focused on cerebellar and limbic pathways yet remains limited by single-timepoint exposure assessment, reliance on questionnaire outcomes, and potential residual confounding from diet and socioeconomic factors.
Regulatory bodies have restricted some phthalates in toys and food packaging; the present findings suggest extending scrutiny to pregnancy-specific exposure routes. Larger, multi-cohort studies with repeated biomarker sampling and direct neurodevelopmental assessments are required to test whether methylation changes predict clinical diagnoses rather than subclinical traits.
Next steps include replication in independent birth cohorts and intervention trials that reduce DEHP exposure during gestation to evaluate reversibility of the epigenetic signature.
Med cohort follow-up: At age 7, children in top exposure quartile will show 1.8-fold higher odds of clinician-confirmed ADHD diagnosis versus lowest quartile (threshold: p<0.05 after adjustment).
