NutriNet-Santé Cohort Links Meat Neu5Gc Intake to 63% Higher Type 2 Diabetes Risk

Preliminary EASD data from a large French cohort associate higher dietary Neu5Gc from red meat with elevated type 2 diabetes risk via chronic inflammation. The link persists after BMI adjustment but remains subject to confounding inherent in observational designs. Confirmation requires prospective biomarker studies and eventual intervention trials.

Researchers used three 24-hour dietary records from the NutriNet-Santé prospective cohort to estimate Neu5Gc exposure from red meat, processed meats, and dairy via a validated food-composition database. Participants without diabetes or cancer at baseline were followed for incident cases confirmed by medical records. The dose-response curve steepened above the fourth quintile, consistent with a threshold for immune incorporation of this nonhuman sialic acid.

After adjustment for age, sex, physical activity, diet quality, smoking, and comorbidities, the hazard ratio remained 1.63; further control for BMI attenuated but did not eliminate the association. The pattern aligns with prior mechanistic work showing Neu5Gc incorporation triggers chronic xenosialitis, a pathway already implicated in cardiovascular disease and cancer within the same cohort.

Because the study is observational, residual confounding by overall meat intake or unmeasured lifestyle factors cannot be excluded. No randomized trial has yet tested whether reducing Neu5Gc specifically lowers diabetes incidence. Next steps require replication in independent cohorts with repeated biomarker measures of circulating anti-Neu5Gc antibodies and longer follow-up to assess microvascular outcomes.

Fezeu et al.: Full NutriNet-Santé 10-year follow-up published by 2028 will show whether anti-Neu5Gc antibody titers mediate at least 30% of the observed diabetes association.
