SNAC-K Observational Data Show 14-24 Hour Meal Gaps Tied to 0.2 Extra Diseases per Year After Age 78
Observational SNAC-K data link longer habitual fasting windows to accelerated multimorbidity after age 78, especially cardiovascular and neuropsychiatric diseases. The association is not causal and may reflect reverse causation or unmeasured confounders. Randomized trials with objective timing measures are needed to clarify directionality.
The Journal of Internal Medicine analysis used repeated measures from the Swedish National Study on Aging and Care in Kungsholmen to track total chronic diseases plus cardiovascular and neuropsychiatric subsets. Meal timing came from single 24-hour recall; models adjusted for age, sex, education, BMI, and physical activity but remained observational. Absolute rates rose from roughly 0.4 to 0.6 new diagnoses annually in the longer-interval group among participants 78 and older.
This pattern runs counter to randomized time-restricted eating trials in middle-aged adults that reported modest cardiometabolic gains, suggesting age-related differences in metabolic flexibility or sarcopenia risk may alter responses. Reverse causation remains plausible: subclinical disease could reduce appetite and extend intervals before diagnoses are recorded. The study did not capture nutrient quality or nighttime eating.
No association appeared for musculoskeletal conditions, hinting at disease-specific pathways. Next steps require objective meal-timing sensors and randomized designs in adults over 75 to test whether shortening intervals slows progression or merely marks better overall health.
VITALIS: An RCT randomizing 75+ adults to 12-hour versus 16-hour daily eating windows will detect no difference in multimorbidity rate at 3 years (hazard ratio 0.95-1.05).
