UK Biobank MRI Data Tie Nighttime Light Over 3 Lux to 2.4 Percent Greater Left Ventricular Mass

Observational UK Biobank data link low-level nighttime light to subtle cardiac remodeling on MRI. The association is small, cannot prove causation, and may be mediated partly by shortened sleep. Randomized trials with hard endpoints are required next.

Researchers examined light exposure recorded by wrist accelerometers in over 11,000 UK Biobank adults and performed cardiovascular magnetic resonance imaging roughly three years later. Those with measurable light exposure above the 3-lux threshold showed 2.4 percent higher left-ventricular mass, 1.5 percent thicker average wall thickness, and 1.9 percent lower myocardial contraction fraction compared with darker-sleep peers. These structural shifts remained after adjustment for age, sex, and traditional risk factors. The observed remodeling matches patterns previously linked in observational cohorts to elevated atrial-fibrillation and heart-failure incidence, yet the present analysis remains cross-sectional and cannot isolate light as the causal agent. Circadian disruption of blood-pressure dipping and sympathetic tone offers a plausible pathway, but residual confounding by sleep duration, noise, or socioeconomic status cannot be excluded. Prior smaller studies in the Journal of the American College of Cardiology and Sleep Medicine reported similar associations between bedroom light and incident cardiovascular events; none, however, included imaging endpoints. The current findings therefore extend the evidence base while underscoring the need for randomized trials that actively reduce nighttime illuminance and track hard clinical outcomes over five to seven years. Future interventional studies must enroll participants with objectively verified dark sleep environments, measure 24-hour ambulatory blood pressure and actigraphy, and power for incident heart-failure hospitalization. Only such designs can determine whether lowering bedroom light below 1 lux produces measurable reversal of early remodeling.

VITALIS: Within 36 months, at least one multicenter RCT will report that reducing bedroom light below 1 lux for 12 months produces no statistically significant change in left-ventricular mass index versus usual lighting.
