US Alcohol-Linked Cancer Deaths Doubled From 1990 to 2023

Alcohol-related cancer deaths doubled in the US over three decades. Multiple studies link even moderate intake to higher risks for several cancers, with absolute increases clearest at heavier consumption levels. Evidence quality is limited by observational data and confounding.

Three recent analyses examined alcohol’s cancer burden using cohort, case-control, and registry data. The Lancet Regional Health – Americas study tracked US mortality trends over 33 years and found increases across most age groups and cancer sites, with colorectal cancer leading alcohol-related deaths in men aged 20–54. A separate meta-analysis of 843 studies published in a Nature journal outlet identified moderate evidence linking any consumption to elevated risk for oral, laryngeal, colorectal, and pharyngeal cancers plus liver disease. A third cohort study in Cancer reported a 25% higher lifetime colorectal cancer risk among heavier drinkers.

These findings align with established mechanisms: acetaldehyde formation, DNA damage, and hormonal disruption. They also extend prior IARC classifications by quantifying recent US mortality growth and highlighting younger adults. Absolute risks remain modest at low intake levels yet rise steeply with volume, consistent with dose-response patterns observed in European and Asian cohorts.

Next steps require updated US cancer registries that capture lifetime drinking history and randomized trials testing whether reduced consumption lowers incidence. Current evidence cannot establish a safe threshold because observational designs leave residual confounding from diet, smoking, and socioeconomic factors unaddressed.
