Danish researchers documented measurable reductions in ovarian and uterine size among infant girls exposed to acetaminophen in utero, consistent with prior animal findings but without individual predictive value. Guidelines remain unchanged while long-term fertility outcomes are tracked. The data force a reassessment of default analgesic recommendations during pregnancy.

The study divided participants by exposure timing: 92 exposed early in pregnancy, 67 later, and 143 unexposed. Mothers reported low-dose use for headaches or pain, never exceeding 4,000 mg daily. At three months, exposed girls showed smaller ovaries and uteruses; a separate cohort of 1,210 girls extended the association to smaller organs at puberty and adolescence.

Animal data had already indicated disrupted ovarian follicle formation from fetal acetaminophen exposure. Current guidelines still list the drug as first-line for pregnancy pain and fever because untreated maternal fever carries documented risks. The Danish findings sit at population level only and cannot forecast individual outcomes, as many exposed daughters showed measurements within the unexposed range.

Health authorities face a classic risk-benefit recalculation: restrict a widely used analgesic or accept possible downstream effects on female reproductive reserve. Long-term tracking of the same cohort through menarche and menopause will determine whether the observed differences translate into earlier ovarian aging or reduced fertility.

No regulatory change has been announced; researchers explicitly state women who used the drug should not be alarmed pending further data.
