CaRi-Heart AI identifies elevated coronary inflammation in 42.9% of asymptomatic adults

Biograph-CaRi-Heart study found coronary inflammation in 42.9% of asymptomatic low-risk adults undetected by bloodwork or risk scores. The finding exposes a gap between conventional screening and vessel-wall biology. Integration of AI inflammation quantification into routine CCTA could alter primary prevention timelines.

The study combined high-resolution CCTA with CaRi-Heart and CaRi-Plaque on clinically eligible Biograph Black members. 42.9% showed elevated inflammation; 17.9% reached very high levels in at least one major artery. 47% of participants with zero detectable plaque still exhibited elevated perivascular fat attenuation, confirming inflammation precedes visible stenosis or calcification.

Traditional risk tools failed entirely. No statistical association existed between inflammation grade and cholesterol fractions, glycemic markers, or the AHA PREVENT calculator. This aligns with prior CRISP-CT and SCOT-HEART substudies where perivascular fat attenuation index independently predicted events beyond plaque burden.

Operationally the result shifts detection upstream. CaRi-Heart, already FDA De Novo authorized, quantifies vessel-wall inflammation from routine scans, enabling earlier statin, anti-inflammatory or lifestyle intervention before irreversible plaque forms. Clinics must now decide whether to add this metric to every CCTA protocol.

Next steps require prospective outcome trials linking CaRi-Heart-guided therapy to event reduction and cost-effectiveness data for payer coverage.

⚡ Prediction

Caristo: 30% of U.S. outpatient CT centers order CaRi-Heart on >50% of CCTA scans within 24 months of payer coverage decisions.
