This structure improves immediate access yet leaves patients exposed to full catastrophic costs after the cap.

Prior observational data in JAMA show GLP-1 persistence falls sharply when out-of-pocket exceeds $150 monthly, suggesting the $50 price may raise initiation rates among lower-income seniors. However, the pilot excludes single-use vials and does not address upstream Part D premium increases or manufacturer rebates, limiting its equity reach. Strict comorbidity requirements may also exclude many with BMI 27-30 who carry prediabetes risk.

Sustainability hinges on utilization monitoring; if enrollment surpasses 10 percent of eligibles by mid-2026, CMS will face pressure to negotiate broader formulary inclusion or extend the program past 2027. Absent such data, the bridge remains a narrow affordability patch rather than systemic pricing reform.
