Augmented Reality Marker-Guided Overlay Used in First Human Liver Tumor Resection June 2026

First-in-human use of marker-based AR overlay for subsurface liver tumors succeeded technically in one case. Evidence remains limited to an uncontrolled feasibility observation. Larger multicenter trials with clinical endpoints and regulatory review are required before efficacy claims can be evaluated.

The intervention combined a preoperative CT scan with real-time camera tracking of a surface marker to compute and display subsurface tumor coordinates. The patient was discharged the following day with no reported complications. As a single-case report without controls or blinded assessment, the observation demonstrates technical feasibility but supplies no data on margin accuracy, operative time reduction, or oncologic outcomes compared with standard intraoperative ultrasound.

Interdisciplinary teams at Chalmers and Sahlgrenska have iterated the geometric registration algorithms for a decade, addressing the smooth-liver surface problem that defeats conventional landmark registration. Comparable AR navigation platforms in neurosurgery and orthopedics have shown initial promise yet frequently encounter soft-tissue deformation errors exceeding 5 mm once clinical series exceed 30 cases, a threshold not yet reached here.

Planned steps include additional liver cases through fall 2026, ISO 13485 quality-system implementation, and a multi-center validation trial with Oslo and Washington sites. Regulatory clearance pathways in Europe and the United States remain undefined; surrogate endpoints such as “successful overlay visualization” will need replacement by clinically relevant measures including R0 resection rates and 30-day morbidity before broader adoption.

⚡ Prediction

VITALIS: No peer-reviewed series reporting more than 25 cases with R0 resection and 30-day morbidity endpoints will appear before Q4 2028.
