The 2024 Chinese cohort of 15,000 men linked extreme temperature exposure to lower semen parameters, while a US couples study found cumulative heat sources modestly reduced conception odds. Cold-water immersion reviews from Australia show benefits for recovery and stress but no data on testosterone or sperm quality. Targeted scrotal cooling differs from whole-body plunges, and no trial demonstrates that post-sauna icing restores spermatogenesis.

These findings separate testosterone claims from actual fertility endpoints. Heat impairs spermatogenesis because testes are normally 2–3 °C cooler than core body temperature; the observed mitochondrial and DNA-packaging shifts provide mechanistic support. Recovery after cessation indicates the effect is transient rather than permanent damage.

Public health messaging should distinguish general wellness gains from fertility-specific evidence. Current data rest on small samples and observational designs; larger randomized trials measuring time-to-pregnancy and DNA fragmentation are required before saunas or cold plunges can be recommended or discouraged for men attempting conception.
