mRNA-4157 Plus Pembrolizumab Cuts Melanoma Recurrence 44 Percent in Phase 3

Phase 3 data confirm that individualized mRNA neoantigen vaccines can meaningfully improve outcomes when added to checkpoint blockade in melanoma. Manufacturing speed and neoantigen prediction accuracy remain the chief barriers to broader application. Larger adjuvant trials across tumor types will determine whether this approach becomes standard care within five years.

The randomized, double-blind KEYNOTE-942 trial enrolled 157 patients after complete resection. Tumor neoantigens were sequenced from each patient’s tissue, then up to 34 mRNA-encoded neoepitopes were manufactured and delivered intramuscularly with the PD-1 inhibitor. At 18 months median follow-up, recurrence-free survival reached 78.6 percent in the combination arm versus 62.2 percent with pembrolizumab monotherapy. The result extends the earlier phase 2b signal and is now being tested in non-small-cell lung, renal, and bladder cancers under the same neoantigen-selection platform.

⚡ Prediction

Moderna: At least two additional tumor types will report positive phase 3 data using the same mRNA-4157 platform by Q4 2026.
