The Colorado Diabetes Registry analyzed continuous, population-level incidence using diagnosis codes, laboratory results, and medication data across health systems for nonmilitary, noninstitutionalized residents under age 20. Denominators came from census figures; joinpoint regression modeled trends separately for type 1 (all <20) and type 2 (10-19). Type 1 incidence edged from 25.07 to 27.39 per 100,000 with APC shifting from 0.66% (2002-2014) to 2.62% (2015-2023). Type 2 showed sustained acceleration: overall APC 3.8%, rising to 4.72% annually after 2014. Obesity-driven diagnostic overlap now blurs classic type distinctions, as high-BMI adolescents may present with either autoimmune or insulin-resistant disease; the SEARCH for Diabetes in Youth consortium has documented parallel national patterns where type 2 incidence now exceeds type 1 in older teens. This trajectory signals an expanding cohort facing decades of microvascular and cardiovascular risk, compounded by documented gaps in pediatric endocrinology access and transition to adult care. Sustained surveillance must incorporate autoantibody and C-peptide testing to refine etiology, while policy responses should target upstream determinants including school nutrition standards and built-environment changes. Next studies require linkage to electronic health records for complication incidence and cost modeling beyond 2030.
