62.5% of Long COVID Participants Meet POTS Criteria in JAHA Observational Comparison
Observational data link long COVID orthostatic tachycardia to POTS diagnostic thresholds in over half of cases examined. Low-grade inflammation and profound work disability appear shared. Larger longitudinal studies with intervention arms are required to test causality and treatment response.
Clinicians can now incorporate a simple 10-minute stand test into long COVID assessments before defaulting to deconditioning explanations. Next steps require prospective cohorts that track symptom trajectories after POTS-specific interventions such as compression garments or ivabradine. An adequately powered RCT with objective activity monitors would clarify whether treating the orthostatic component alters fatigue or cognitive scores.
Adelaide University: A 200-participant prospective cohort will show that stand-test positivity at six months post-infection predicts failure to resume full-time work at 18 months with >70% positive predictive value.
