Endoscopic Sleeve Gastroplasty Delivers Durable Loss After GLP-1 Plateau in Former Athlete
Personal narrative of post-GLP-1 weight regain illustrates ESG as a minimally invasive bridge for refractory obesity. Data from RCTs and registries indicate 12-15% sustained loss with ESG, offering an alternative when pharmacotherapy plateaus or is discontinued. Multidisciplinary long-term follow-up remains essential to prevent recidivism.
Bichler, a former competitive weightlifter, reached nearly 300 pounds despite repeated dieting and sequential GLP-1 trials; each produced initial loss followed by plateau and regain. Fatty liver disease with family history of cirrhosis prompted referral to University of Minnesota for ESG, which reduces gastric capacity via endoscopic suturing without resection or incisions. Patients typically resume activity within days, contrasting longer recovery after surgical sleeve gastrectomy. Observational cohorts and one RCT (Gastroenterology 2022) show mean total body weight loss of 12-15% at 12 months with ESG, with 60-70% retention at 3 years in completers. In contrast, STEP-1 extension data indicate 2/3 of semaglutide-induced loss returns within one year of discontinuation, underscoring the durability gap when pharmacotherapy cannot be maintained. ESG therefore fills a niche for patients intolerant or refractory to ongoing GLP-1 exposure. Multidisciplinary programs combining ESG with behavioral support report lower 5-year regain than either modality alone. Remaining questions include head-to-head RCTs versus continued GLP-1 or combination regimens, long-term liver histology outcomes, and cost-effectiveness thresholds relative to lifetime medication expense.
ESG volume among GLP-1 non-responders will rise above 15% of bariatric endoscopy cases within 24 months at academic centers.
