LITMUS Trials Show Gamma-GT and FibroScan Can Replace Biopsies for Advanced MASLD Fibrosis

Large LITMUS studies in The Lancet and Nature Medicine validate gamma-GT, ELF, and elastography for advanced fibrosis detection in MASLD, supporting biopsy-sparing pathways. Inflammation assessment and outcome prediction require further validation before full regulatory substitution.

The LITMUS consortium evaluated dozens of blood and imaging biomarkers against expert pathologist reads of liver biopsies in cohorts exceeding 1,500 and 350 participants. For advanced fibrosis and cirrhosis, SOMAScan, MRE, and FibroScan-based scores exceeded performance targets; gamma-GT alone surpassed FIB-4 as an initial screen. At-risk MASH detection remained weaker, with only the proteomic panel crossing the prespecified bar. These prospective, independent data establish realistic benchmarks for tests already available in routine labs and clinics.

Rising MASH drug approvals create immediate pressure to triage patients without biopsy. A gamma-GT-first pathway followed by ELF or FibroScan could cut specialist referrals and invasive procedures while preserving accuracy for cirrhosis, the stage driving most liver-related mortality. Regulators still require histologic confirmation for trial enrollment, so biopsy use will persist in late-phase studies even as routine care shifts. Health systems adopting the pathway will need local validation of cut-offs and reimbursement alignment.

Remaining gaps include inflammation detection and longitudinal monitoring. Next studies must test whether serial noninvasive scores predict clinical outcomes such as decompensation or HCC incidence over five years, and whether they can serve as primary endpoints in phase 3 trials.

⚡ Prediction

EMA: By end-2027 at least one elastography or blood panel will be incorporated into official MASLD referral guidelines with performance thresholds matching LITMUS benchmarks.
