
Cohort Analysis Associates One Daily Sugary Drink With 2.45-Fold Stomach Cancer Risk
Daily sugar-sweetened beverage intake was associated with elevated stomach cancer risk in large U.S. cohorts, while artificially sweetened drinks were not. The observational design limits causal claims and highlights the need for replication and mechanistic work.
Researchers tracked diet via repeated food-frequency questionnaires over decades in two U.S. cohorts, identifying 205 incident gastric cancers. The hazard ratio remained elevated after adjustment for smoking, BMI, physical activity, and Helicobacter pylori proxy variables, though absolute risk stayed low given the cancer’s baseline incidence of roughly 7 per 100,000. Artificially sweetened beverages showed no association, consistent with differing caloric and fructose-mediated pathways.
Prior work from the same group has tied sugary drinks to colorectal, breast, and liver cancers, suggesting shared metabolic routes via insulin resistance and visceral adiposity. The current findings extend this pattern to gastric adenocarcinoma, a malignancy often detected late without routine screening. Limitations include self-reported intake, residual confounding by unmeasured dietary patterns, and predominantly White, health-professional participants.
Future replication in diverse populations and mechanistic studies measuring serum fructose and microbiome shifts are required before causal inference or dietary guidelines can be updated. Randomized substitution trials would strengthen the evidence but face adherence challenges.
