FDA Clears Updated 2026-27 COVID-19 Vaccines Targeting XFG Variant for High-Risk Groups

On August 27, 2026, the U.S. Food and Drug Administration approved updated COVID-19 vaccines from Pfizer/BioNTech (Comirnaty), Moderna (Spikevax and mNEXSPIKE), and Novavax/Sanofi (Nuvaxovid) for the 2026-2027 season. The monovalent shots target the XFG subvariant of the JN.1 lineage, selected by the FDA's Vaccines and Related Biological Products Advisory Committee in May 2026 to better match circulating strains.[1][2]

Eligibility remains targeted: adults 65 and older, plus individuals ages 5–64 (or as young as 6 months for one Moderna formulation) with underlying conditions such as obesity, diabetes, heart/lung/kidney disease, or pregnancy that elevate severe disease risk. Pfizer and Moderna vaccines use the mRNA platform; Sanofi/Novavax offers the sole protein-based (non-mRNA) option. Approvals relied on prior clinical data, immunogenicity from animal and prior human studies, and manufacturing information rather than new large-scale placebo-controlled trials in humans for this update—citing operational challenges. CDC data indicate COVID-19 activity is growing or likely growing in 47 states amid low prior-season uptake (17.5% adults, 10% children).
