Large observational evidence from 3.2 million births demonstrates conventional fetal growth charts produce unstable SGA classification across English regions due to lack of maternal adjustment. Customized GROW charts deliver consistent detection aligned with local demographics. National standardization and prospective outcome-linked validation are now required to reduce avoidable stillbirth.
Next steps require a coordinated NHS program with mandatory chart standardization and prospective ultrasound validation against perinatal mortality. Without such oversight, variation in detection thresholds will persist and continue to drive preventable adverse outcomes. A national audit of actual ultrasound-estimated fetal weight trajectories versus birth-weight benchmarks is the logical follow-on study to quantify true diagnostic performance.
VITALIS: By December 2027 at least 60% of English NHS trusts will have switched to GROW charts under national oversight or face a CQC review trigger.
