Danish Ultrasound Cohort Links First-Trimester Paracetamol to 12% Smaller Newborn Gonadal Volume

An observational ultrasound study of 214 newborns found smaller gonads after early paracetamol exposure but could not establish causation. Confounding by maternal illness remains plausible and untested by stronger designs. Larger registry studies with indication adjustment are required before policy changes.

Copenhagen University Hospital researchers used 3D ultrasound to measure gonadal volume in 67 exposed and 147 unexposed newborns. Exposure came from maternal diaries covering weeks 8-12; volumes differed by roughly 12% after adjusting for birth weight and sex. No hormone assays or follow-up beyond the neonatal period were performed.

Prior work from the same group linked the same exposure window to earlier pubertal timing in girls. The current findings extend that signal to structural measures but cannot isolate drug effects from maternal fever, infection, or socioeconomic factors that prompt analgesic use. Prescription-registry and sibling-comparison designs would be needed to reduce this bias.

Regulatory bodies have already tightened third-trimester advice; extending caution to the first trimester would require replicated dose-response data in cohorts exceeding 1000 pregnancies with verified indication. Without such evidence, clinical guidance should continue to emphasize the lowest effective dose for the shortest time.

Copenhagen team: In a 1000-pregnancy replication using prescription records and sibling controls, the gonadal-volume difference will fall below statistical significance once maternal indication is adjusted.
