Exercise Physiology Data Show Menstrual Cycle Phase Has No Measurable Effect on Vascular or Muscle Outcomes

Longstanding exclusion of women from exercise studies rested on an untested 1995 claim of menstrual-cycle interference. Direct measurements across cycle phases found no meaningful differences in vascular or strength outcomes. This opens a path for sex-specific guidelines but highlights decades of male-only data still embedded in clinical recommendations.

The team tested endothelial function, arterial stiffness, and resistance-training adaptations at multiple cycle phases in cohorts of naturally cycling women and those using combined oral contraceptives. Absolute changes in flow-mediated dilation stayed within 0.5 percentage points across phases; one-repetition-maximum gains differed by less than 3 percent between groups. These null results were replicated in subsequent studies of substrate metabolism and recovery markers.

The exclusion practice originated in a 1995 paper that reported cycle-related variability, yet later meta-analyses of more than 1,200 women showed effect sizes below 0.2 for most performance variables. This mirrors the earlier cardiology shift after sex-stratified trials revealed different myocardial infarction symptom profiles and outcomes in women versus men.

Because the assumption lacked empirical support, entire domains of exercise prescription for women rest on male-derived data. Current guidelines for hypertension, sarcopenia, and athletic training therefore carry unknown sex-specific error margins.

Next steps require adequately powered, cycle-controlled RCTs that report absolute risk reductions separately by sex and include diverse hormonal-contraceptive regimens to close the evidence gap within five years.
