Life Biosciences administers first partial OSKM reprogramming dose to glaucoma patient

Life Biosciences opened the first human OSKM reprogramming trial for glaucoma. The approach resets cellular epigenetic state rather than targeting individual aging hallmarks. Funding concentration in three reprogramming companies now exceeds $5 billion.

The trial targets optic-nerve regeneration by transient expression of Oct4, Sox2, Klf4 and c-Myc. Prior mouse studies demonstrated restored axon density and visual evoked potentials after 2–4 weeks of doxycycline-inducible OSKM delivery without full dedifferentiation. The protocol limits factor expression to avoid teratoma formation observed in full reprogramming regimens.

Lopez-Otin et al. (Cell, 2013) listed nine hallmarks; telomere attrition and senescence each dominated sequential funding cycles before 2020. Reprogramming instead acts on the epigenetic maintenance module, resetting DNA-methylation clocks and histone marks rather than clearing p16-positive cells or extending telomeres. This mechanistic distinction explains why Altos Labs raised $3 billion and Retro Biosciences reached $1.8 billion valuation after $180 million from Altman.

NewLimit’s planned 2027 liver trial will measure H3K9me3 and methylation age in hepatocytes as primary endpoints. Success thresholds require at least 3-year epigenetic reversal sustained at 12 months post-dose. Regulatory precedent from ocular gene therapy may compress IND timelines for subsequent reprogramming indications.

Operational risk centers on vector immunogenicity and off-target gene activation; first-in-human safety data will determine whether partial reprogramming advances beyond ophthalmology.
