Micro-arousals during sleep linked to higher polygenic Alzheimer's risk only in adults aged ~59, SLEEP observational study finds

Observational polysomnography data link more frequent micro-arousals to higher Alzheimer's polygenic risk solely in late-middle age. The cross-sectional design shows association but cannot establish directionality or clinical utility. Longitudinal biomarker studies are required to test whether sleep fragmentation precedes or reflects early locus coeruleus pathology.

Researchers at the University of Liege recorded overnight polysomnography in 501 volunteers without cognitive impairment and calculated each participant's polygenic risk score for Alzheimer's. They quantified brief EEG arousals that fragment sleep without full awakening. The association between higher arousal index and genetic risk reached statistical significance only in the older subgroup; no relationship appeared in younger adults. The study measured neither amyloid nor tau deposition directly.

The locus coeruleus, an early site of tau accumulation, regulates arousal and is implicated by the authors as a plausible mediator. This finding aligns with prior observational data showing sleep fragmentation precedes measurable cognitive decline by years, yet the current cross-sectional design cannot distinguish whether arousals accelerate proteinopathy or simply reflect early brainstem involvement. Absolute differences in arousal rates were modest and overlap between high- and low-risk groups was substantial.

Because the study is observational, reverse causation and residual confounding remain possible. It supplies no evidence that treating arousals alters Alzheimer's incidence. Next steps require longitudinal cohorts that track both sleep metrics and biomarker trajectories over at least five years, plus Mendelian randomization or intervention trials targeting sleep continuity to test causality.

Future work must also determine whether the observed association is specific to Alzheimer's polygenic scores or appears with other neurodegenerative risk profiles.

⚡ Prediction

VITALIS: In a 5-year follow-up of this cohort, participants above the 75th percentile for both micro-arousals and PRS will show at least 0.3 SD greater annual decline on the Preclinical Alzheimer Cognitive Composite than low-risk peers.
