The interrupted time-series analysis in CMAJ examined August 2020-March 2025 data across BC, Ontario and Alberta, capturing 97,630 possession encounters and 29,730 toxicity deaths. Decriminalization produced an immediate, sustained drop in possession incidents that returned to baseline after May 2024 public-space restrictions; trafficking encounters and hospitalizations remained stable relative to other provinces. National declines in deaths during the recriminalization window occurred in control provinces as well, indicating broader supply or naloxone trends rather than the BC policy shift.

The study isolates one narrow mechanism: removal of criminal penalties for ≤2.5 g possession measurably reduced documented police contact without altering the toxic drug supply driving mortality. This aligns with Portugal's decriminalization experience, where possession arrests fell sharply yet overdose trajectories required separate safe-supply and treatment investments. BC's results therefore highlight that decriminalization functions as a harm-reduction tool for stigma and enforcement contact, not a standalone mortality intervention.

Observational design limits causal claims; unmeasured changes in drug potency, fentanyl distribution, or COVID-era reporting could confound outcomes. The next required evidence is a controlled comparison of jurisdictions adopting decriminalization plus pharmaceutical-grade supply versus decriminalization alone, with pre-specified primary endpoints of fatal and non-fatal overdose rates over at least three years.
