Audible pops during spinal manipulation show no link to superior pain relief in 2022 review of 303 adults
Back cracking sounds arise from tribonucleation in facet joints but do not predict better pain relief. Studies show expectations shape perception more than the pop itself. High-quality RCTs isolating sound are still needed.
The MedicalXpress piece correctly notes that tribonucleation creates the cracking sound when facet joint surfaces separate rapidly, forming a gas cavity visible on MRI in knuckle studies. Yet it underplays how patient expectations amplify perceived force, as shown when headphone-played creaky sounds altered force estimates despite identical pressure. This cognitive overlay explains why 49% of surveyed patients equate the pop with vertebral repositioning, a belief unsupported by imaging.
The 2022 review aggregated 303 adults and isolated the sound variable; noisy and silent manipulations produced statistically indistinguishable pain reductions. Self-cracking lacks comparable trials, but the pattern aligns with manipulation data where movement itself, not acoustics, drives short-term stiffness relief. Observational designs cannot rule out placebo from ritualized twisting.
Future work requires sham-controlled RCTs measuring both immediate range-of-motion and 4-week pain scores while blinding participants to sound, testing whether expectation alone accounts for relief reports.
VITALIS: Next sham-controlled RCT on self-stretching will find audible sounds associated with <10% additional pain reduction at 4 weeks versus silent controls.
