NRG-HN009 Phase II Shows Weekly Cisplatin Matches Q3 Toxicity but Raises Locoregional Failure in p16-Negative Disease
NRG-HN009 Phase II RCT demonstrates equivalent acute toxicity burden between cisplatin schedules in p16-negative head and neck cancer with a numerical increase in early locoregional failure for weekly dosing. The findings halt further evaluation of the weekly arm in this population and underscore the need for individualized regimens.
These results close the p16-negative cohort to phase III expansion. Future trials must test biomarker-guided dosing or novel radiosensitizers rather than schedule adjustments. Direct comparison of progression-free survival at two years and late toxicity will determine whether any subgroup still warrants weekly administration.
