Retrospective Chinese case-control study links higher FeNO and small-airway dysfunction to pediatric CTVA; associations with atopy and obesity are noted but causality remains unproven. Larger prospective studies with diagnostic accuracy endpoints are needed.

Combination of FeNO with small-airway indices may aid recognition of CTVA, but prospective diagnostic accuracy studies with blinded adjudication and standardized provocation testing are required before routine adoption. Such trials should report sensitivity, specificity, and net reclassification improvement against clinical diagnosis.

VITALIS: Within 24 months a multicenter prospective cohort will report FeNO plus MEF50 sensitivity above 75% for CTVA diagnosis at a predefined 35 ppb threshold.
