BCI Human Implants Exceed 100 Subjects by June 2026 Across Neuralink, Synchron, Neuracle

BCI trial volume has tripled since the 2024 benchmark of 67 participants, driven by regulatory firsts and wireless hardware. Medical deployment now depends on three-year reliability metrics rather than proof-of-concept demonstrations. The pattern mirrors cochlear-implant adoption curves from the 1990s.

The shift began with the 2024 review covering 21 research groups and 67 participants from 1998-2023. Since then Neuralink reached 21 implants, Synchron expanded trials across North America and Australia, and Neuracle secured NMPA approval for routine use outside trials after its November 2024 start. Wireless and high-density arrays replaced earlier tethered systems, directly raising channel counts and decoding stability in ALS and spinal-cord injury cohorts.

Data now show functional gains beyond lab metrics. Harrell’s UC Davis device reached phoneme-level accuracy sufficient for daily web navigation and paid work after three years of iterative calibration. Parallel cohorts report cursor control at 90-plus percent accuracy and independent device operation without constant caregiver input. These outcomes track electrode proximity rather than company branding, confirming the 2024 review’s invasiveness trade-off predictions.

Operationally, regulatory clearance in China and active IDE filings in the US compress the timeline from first-in-human to reimbursed deployment. Hospitals must now budget for surgical teams, wireless telemetry maintenance, and profanity-filter software updates previously treated as research extras. Next-phase scaling hinges on 2026-2027 durability data from the first 50 wireless implants rather than additional volunteer recruitment.
