Observational Analysis of 2 Million Surgical Cases Links GLP-1 Use to 11% Lower Cardiovascular Complications

Large observational data link GLP-1 agonists to modestly lower postoperative heart complications and infections. Benefits likely stem from metabolic and anti-inflammatory effects seen in prior CVOTs, but causality remains unproven. RCTs are essential to guide perioperative management.

Researchers queried the Epic Cosmos database for adults undergoing procedures longer than 60 minutes with at least 24-hour stays from 2016-2024. Of 145,610 GLP-1 users, matched analysis showed reduced rates of myocardial infarction, stroke, cardiac injury, and death, plus fewer surgical-site, urinary, pulmonary, and bloodstream infections. Proposed mechanisms include improved glycemic control, reduced inflammation, and hemodynamic stability during surgical stress.

This pattern aligns with cardiovascular outcome trials such as SELECT and SUSTAIN-6, where semaglutide lowered major adverse cardiac events by 20% in nonsurgical populations. Perioperative data remain sparse; prior small studies reported conflicting signals on aspiration risk and delayed gastric emptying, leaving anesthesiologists without clear perioperative protocols.

The study is observational and cannot establish causality; residual confounding by healthier user behavior or better overall diabetes management is possible. Randomized perioperative trials measuring hard endpoints are required before guideline changes.

Evidence quality is moderate: large sample size and matching strengthen associations, yet selection bias and lack of randomization limit causal inference; next steps demand multicenter RCTs with standardized GLP-1 continuation protocols.

⚡ Prediction

VITALIS: Within 36 months, at least one multicenter RCT of perioperative GLP-1 continuation will report a primary endpoint showing ≥8% absolute reduction in 30-day MACE or infection composite.
