New York Nursing Homes Lose 269 Beds as National Capacity Drops 5% Since 2019
Nursing home bed scarcity stems from chronic Medicaid underpayment and staffing gaps, producing measurable capacity losses in New York and nationally. Studies in JAMA Internal Medicine and NEJM document a 5% decline since 2019 coinciding with rising demand from aging demographics. Without reimbursement reform, closures and waiting lists will expand.
ArchCare, operating under the Archdiocese of New York, reduced beds from 120 to 53 at its South Bronx site and closed a full floor on Staten Island while shrinking its Dutchess County home from 309 to 196 beds. Statewide, 24 homes closed since 2020, eliminating over 3,000 licensed beds. Nationally, CMS-certified facilities fell from 15,648 in 2015 to under 14,750 by 2025.
Medicaid, paying below daily care costs, drives the contraction according to ArchCare and industry groups. Short-stay Medicare patients cannot offset losses at most facilities. Staffing shortages persist, with aide numbers below 2015 levels despite partial pandemic recovery and median wages of $20.67. Occupancy at 80% masks operating-bed constraints from understaffing.
Demographic pressure intensifies as baby boomers turn 80, raising long-term care demand sharply. Grabowski's JAMA analysis and Mor's NEJM piece both flag accelerating shortfalls, worsened by potential Medicaid cuts. Low Medicaid rates and aide turnover create a structural mismatch between licensed and functional capacity.
Facilities will likely continue selective closures unless states raise Medicaid rates above cost. Researchers project further 3-7% capacity erosion by 2028 without intervention, lengthening wait times for frail elderly patients.
