Ontario gambling disorder ED visits rose 154% above expected for males aged 10-29 after 2022 private online betting expansion

Canada legalized single-event sports betting in August 2021. Ontario opened its online market to private firms in April 2022, resulting in 83 sites and $82.7 billion CAD wagered in 2024. Researchers at the University of Toronto examined 14 years of ED data for gambling disorder codes across ages 10-105, recording 952 visits among 757 individuals, three-quarters male. The design compared observed versus expected visit counts before and after market expansion.

Post-expansion counts exceeded projections by 154 percent for males 10-29 and 116 percent for males 30-44, with no detectable change among females. Over 70 percent of visits involved comorbid mental health or substance-use diagnoses, and nearly one-third resulted in admission. These ED trends align with earlier ConnexOntario helpline increases but provide independent confirmation because advertisements do not direct users to emergency departments.

The concentration in young males tracks targeted sports-betting promotion and rapid account growth to 2.6 million. Prior studies show most individuals with gambling problems avoid formal care until acute harm occurs, implying the observed ED surge understates total population burden. Alberta's July 2026 private-operator launch creates a natural experiment for replication.

Other provinces face similar legislative pressure. Provincial health systems should prepare for sustained rises in acute presentations by expanding brief intervention capacity in EDs and mandating real-time harm-monitoring dashboards tied to operator data feeds.
