Large review finds leisure but not occupational physical activity protects against dementia. Domain-specific risks appear driven by education, pollution, and recovery differences rather than movement volume. More domain-stratified cohorts in low-income settings are required.

The review aggregated prospective cohorts and case-control data to examine domain-specific physical activity. Leisure activity such as gym exercise or sports showed consistent inverse associations. Occupational activity, drawn from five studies, produced a pooled hazard ratio indicating elevated risk. Active commuting from two studies showed a smaller 10% increase. Researchers adjusted for age, sex, and education but noted residual confounding from unmeasured workplace exposures remained possible.

Contextual factors likely drive the paradox. Physically demanding jobs often coincide with lower educational attainment, reduced cognitive reserve, and higher exposure to air pollution, noise, and heat. Leisure exercise occurs in controlled settings with recovery time, whereas occupational demands frequently involve prolonged static postures or repetitive strain without equivalent cardiovascular or neuroprotective benefits.

Prior UK Biobank accelerometer data and the English Longitudinal Study of Ageing reported dose-dependent protection from moderate-to-vigorous activity, yet neither stratified by domain. The new synthesis reveals that volume alone does not determine outcomes; setting and co-exposures matter. Low-income countries, where recreational activity comprises only 4% of total movement, remain underrepresented.

Future studies must incorporate personal air-quality monitors, job-exposure matrices, and repeated cognitive assessments in diverse economic settings. Randomized workplace interventions that reduce pollution or add recovery breaks could test causality.

VITALIS: By 2028, at least one prospective cohort of 200,000+ workers in low- and middle-income countries will report that adjustment for fine-particulate exposure eliminates the occupational activity-dementia association.
