The Notre Dame study in npj Precision Oncology marks the first targeted genomic profiling of breast tumors from Native American women, contrasting 17 cases against 700 TCGA samples dominated by European ancestry. While incidence is lower, mortality remains stubbornly elevated, a pattern long attributed to delayed diagnosis yet now linked to molecular distinctions in immune evasion and DNA repair pathways. Tumors exhibited elevated NOTCH4 nuclear localization and mutations in immune-recognition genes absent or rare in white cohorts, suggesting differential responses to checkpoint inhibitors and PARP inhibitors. This observational genomic analysis, limited by its tiny Native sample and lack of matched controls for stage or treatment, cannot establish causality but challenges the assumption that TCGA-derived biomarkers generalize. Prior work, including a 2021 JAMA Oncology review of SEER-linked ancestry data (n>50,000), documented similar underrepresentation skewing risk models, while a 2023 Nature Communications paper on admixed populations highlighted immune-gene variants enriched in Indigenous groups. Socioeconomic confounders persist unmeasured here; the study correctly flags them yet underplays how historical exclusion from biobanks compounds both biological and access barriers. Expanding the Harper repository offers a corrective, but power calculations indicate hundreds more samples are needed before altering guidelines.
