Observational Study of 198 Adults Links Higher Fitness to Slower eGFR Decline Over Five Years
Observational data from 198 adults show higher cardiorespiratory fitness associated with slower five-year eGFR decline after adjustment for confounders. The finding is consistent with broader fitness-CKD literature yet remains limited by design and sample size. Randomized exercise trials are needed to test causality and clinical impact.
Researchers in Ibaraki Prefecture measured cardiorespiratory fitness via submaximal exercise testing and tracked estimated glomerular filtration rate annually for five years in participants who started with normal kidney function. Higher fitness levels correlated with reduced annual eGFR loss and lower odds of a 20 percent or greater clinically meaningful drop, independent of age, sex, body composition, smoking, comorbidities, and baseline values. The study adds to evidence that endurance capacity may influence organ aging trajectories in middle-aged and older adults.
Japan’s rapidly aging population faces rising chronic kidney disease prevalence, and this work connects cardiorespiratory fitness to kidney preservation in a way earlier cross-sectional surveys could not. However, unmeasured factors such as dietary protein intake, sleep patterns, and specific exercise modalities remain unaccounted for, and the modest sample size limits subgroup analyses by sex or comorbidities. The observational design cannot distinguish whether fitness directly protects nephrons or merely marks healthier lifestyles.
Prior larger cohorts, including a 2019 JAMA Internal Medicine analysis of over 20,000 adults, reported similar inverse associations between fitness and incident CKD but lacked serial fitness testing. Randomized trials are now required to test whether structured aerobic interventions can measurably slow eGFR trajectories and reduce dialysis incidence. Future studies should incorporate objective fitness reassessment and longer follow-up to establish dose-response relationships.
Regulatory bodies and public health agencies could incorporate cardiorespiratory fitness screening into CKD prevention guidelines if interventional data confirm benefit, potentially extending healthy life expectancy in aging societies.
VITALIS: A 2027-initiated multicenter RCT of 800 adults aged 50-75 will show that a supervised 150-minute weekly aerobic program slows eGFR decline by at least 8 percent versus usual activity at three-year follow-up.
