mRNA melanoma vaccine cuts recurrence risk 44% in phase 2b trial; FDA decision due 2025

Personalized mRNA vaccine plus PD-1 blockade improved recurrence-free survival in resected melanoma. Phase 3 data and manufacturing scale-up will determine whether approval follows in 2025. Evidence strength is moderate pending larger trials.

The Moderna-BioNTech candidate mRNA-4157 encodes up to 34 patient-specific neoantigens identified from tumor exome sequencing. After surgical resection, participants received up to nine intramuscular doses alongside standard pembrolizumab. Recurrence-free survival at 18 months reached 78.6% in the combination arm versus 62.2% with pembrolizumab monotherapy, with manageable reactogenicity.

Prior neoantigen vaccine efforts relied on dendritic-cell or peptide platforms that proved logistically cumbersome and weakly immunogenic. The mRNA format enables rapid manufacturing from sequencing data and leverages lipid nanoparticles for direct cytosolic delivery, addressing two historical bottlenecks. Real-world rollout will still require scalable tumor sequencing pipelines and cold-chain infrastructure.

The ongoing phase 3 trial targets 1,089 participants with broader inclusion; success hinges on replicating the hazard ratio while confirming overall-survival benefit. Regulatory precedent from the 2023 accelerated-approval pathway for other mRNA products suggests a 2025 decision is plausible if the primary endpoint is met.

Key limitation remains the modest sample size and short follow-up of the phase 2b data; larger randomized evidence with mature survival endpoints is required before routine adoption.

⚡ Prediction

FDA: full approval granted by December 2025 if phase 3 recurrence-free survival HR remains below 0.65 at 24 months
