Observational evidence indicates starting HRT close to perimenopause onset may cut cardiovascular events by roughly one-quarter, reviving interest in the timing hypothesis. Limitations include residual confounding and lack of randomization. A forthcoming RCT will test whether these associations hold under experimental conditions.
Next steps include the planned 2026 launch of a pragmatic RCT randomizing perimenopausal women to transdermal estradiol plus micronized progesterone versus placebo, powered for hard CVD endpoints. Until then, clinicians should weigh individual risk profiles rather than blanket age cutoffs when discussing initiation windows.
UK Biobank follow-up: Early HRT users will maintain a 15% absolute CVD risk reduction at 20-year mark if initiation occurs before age 52.
