Survey of 1,800 women links persistent GI symptoms to elevated anxiety across all menopause stages

Cross-sectional survey shows consistent GI symptom burden and stage-specific anxiety elevation in menopause. Evidence remains observational and self-reported; longitudinal microbiota data may clarify directionality. Screening for both domains is reasonable pending stronger causal studies.

The study collected self-reported GI and anxiety data from more than 1,800 participants stratified into pre-, peri-, and postmenopause groups. Researchers used validated symptom severity scales but did not assess hormone levels, diet, or medication use, limiting ability to isolate mechanisms. Bloating remained elevated throughout transition, unlike anxiety which peaked during perimenopause, suggesting the gut-brain interaction may operate independently of stage-specific hormone fluctuations.

Conference presentation at the 2026 Menopause Society meeting omitted key details on recruitment bias and response rate, both common issues in menopause surveys that often over-represent symptomatic women. Prior observational work in JAMA and The Lancet has shown similar GI-anxiety clustering but relied on smaller samples and lacked longitudinal follow-up, leaving open whether symptoms precede or follow anxiety onset.

Next analyses will incorporate gut microbiota sequencing from year-two cohort visits to test for microbial mediators. Without randomized intervention data or objective motility measures, clinicians should interpret these associations as hypothesis-generating rather than actionable for targeted therapy.

⚡ Prediction

MOMENTUM investigators: microbiota diversity metrics will correlate with GI symptom scores at r>0.25 in the year-two longitudinal sample of at least 1,200 participants.
