NEJM RCT: CGM Adds 5 Hours Daily in Target Range for Non-Insulin Type 2 Diabetes

Multicenter RCT demonstrates clinically meaningful glycemic improvement with CGM in non-insulin type 2 diabetes treated in primary care. Findings support upgrading evidence level for guidelines and coverage decisions. Six-month duration limits inference on sustained outcomes and complication reduction.

The trial randomized 283 adults across 22 U.S. primary care practices to CGM or standard fingerstick monitoring. Primary endpoint was time in range (70-180 mg/dL) via masked CGM at 26 weeks; secondary measures included A1c change and proportion reaching A1c below 7%. CGM users achieved 5 more hours in range and a 0.9-point greater A1c reduction, with nearly twice as many meeting the A1c goal.

This population of roughly 20 million U.S. adults has historically received only Level C guideline support because prior CGM data came mainly from insulin users or small single-site studies. By enrolling exclusively in primary care settings, the trial directly addresses the site where most type 2 diabetes care occurs and provides Level A-grade evidence that could prompt ADA guideline revision and broader insurance coverage.

Remaining questions include durability beyond 26 weeks, cost-effectiveness thresholds for payers, and whether CGM-driven behavior change persists after sensor removal. A follow-up trial with 52-week outcomes and healthcare utilization endpoints would strengthen policy impact.

Longer-term observational data from integrated health systems will be needed to confirm reductions in microvascular complications before CGM becomes first-line for all non-insulin type 2 diabetes.

⚡ Prediction

VITALIS: ADA Standards of Care 2027 will list CGM as Level A recommendation for non-insulin type 2 diabetes if a second 12-month RCT reports consistent time-in-range gains above 2 hours.
