Swedish Regional Data Show Mixed, Non-Significant Links Between Folic Acid Dispensing and Youth Suicide Deaths

An ecological Swedish study of 3,463 deaths found inconsistent, non-significant associations between folic acid dispensing and suicide mortality by age and sex. Results highlight confounding risks in observational data and call for individual-level longitudinal studies before any policy change.

The study, presented at the 2026 ECNP congress, compared regional folic acid dispensing rates with suicide and undetermined-intent deaths across Sweden's 21 regions. Among men aged 20-24, higher dispensing correlated with elevated suicide-related mortality; among men aged 25-29 the association reversed. No consistent signal appeared in women. Researchers explicitly cautioned against causal interpretation, noting the ecological design cannot separate treatment effects from prescribing indications.

This contrasts with the 2022 JAMA Psychiatry observational analysis that reported a 44% lower rate of suicidal events during periods covered by folic acid prescriptions. Both studies remain vulnerable to confounding by indication: folic acid is often prescribed for anemia, pregnancy planning, or psychiatric comorbidity, any of which may independently influence suicide risk. The Swedish findings underscore that regional aggregates cannot adjudicate individual-level causality.

Next steps require longitudinal cohorts that track prescription timing, clinical indications, and psychiatric history at the patient level, ideally followed by randomized trials powered for suicide attempts. Until such data exist, public-health recommendations on folic acid for suicide prevention lack sufficient support.

⚡ Prediction

Jokinen: No individual-level cohort study published by 2029 will demonstrate a causal 20% or greater reduction in suicide attempts attributable to folic acid after adjustment for psychiatric indication.
