Meta-analysis of 43,000 Women Shows No Clear Rise in Major Pregnancy Complications With GLP-1 RA Exposure
Meta-analysis of GLP-1 RA use in pregnant women with diabetes finds no increase in most major adverse outcomes but highlights critical evidence gaps around miscarriage and later-trimester exposure. Authors call for urgent prospective studies as prescribing accelerates.
The analysis, prepared for the 2025 EASD meeting and published in The Lancet Obstetrics, Gynaecology & Women's Health, pooled data from observational cohorts that captured GLP-1 RA dispensing records linked to birth registries. Six of the seven studies focused on first-trimester exposure; timing was often recorded only to the month. Absolute risks for congenital anomalies and neonatal death remained statistically indistinguishable from unexposed women with type 2 diabetes, though confidence intervals were wide. Early-pregnancy loss appeared 31 percent higher, yet the dominant study merged miscarriage and elective termination into a single endpoint, preventing separation of spontaneous loss from patient choice. Observational designs cannot exclude residual confounding by obesity severity or glycemic control, and only one study examined exposure beyond the first trimester. These gaps matter because GLP-1 RA prescriptions to reproductive-age women are rising rapidly while animal data suggest possible effects on implantation. The international consensus statement therefore recommends discontinuation at least one month before planned conception until better data arrive. Future work must include prospective pregnancy registries that record exact exposure weeks, distinguish miscarriage from termination, and follow offspring for metabolic outcomes. An ongoing Scandinavian registry linkage aims to deliver such data by 2027.
