260000-person cohort shows WHR reclassifies 18% of normal-BMI adults to elevated CVD risk

Large cohort data demonstrate waist-to-hip ratio reclassifies cardiovascular risk beyond BMI alone. Normal-weight adults with elevated central adiposity face 15-50% higher event rates. Clinical protocols must incorporate circumference measures to reduce misclassification.

The study added waist circumference and waist-to-hip ratio to BMI categories in nine pooled cohorts. Among normal-weight participants, 5% had high waist circumference and 18% had high WHR; both markers correlated with increased myocardial infarction, stroke, heart failure, atrial fibrillation, and mortality. Obesity with low WHR showed no excess risk versus normal weight with low WHR except lower all-cause mortality.

Visceral fat metrics capture inflammatory and metabolic pathways BMI misses. Prior UK Biobank and Framingham analyses reported similar reclassification rates but lacked the nine-outcome granularity or 20-year span. The single baseline measurement and absence of activity or genetic covariates limit causal inference on change over time.

Primary-care panels that retain BMI-only thresholds will continue to under-identify risk in normal and overweight strata while over-treating some obese patients with low central adiposity. Operational shift requires integrating tape-measure WHR into intake protocols and updating risk calculators within two years.

Guideline committees will test WHR thresholds against existing pooled-cohort equations; adoption above 20% of US practices by 2028 would trigger electronic health record default changes.

AXIOM: US primary-care practices will add WHR to intake forms in at least 15% of sites by end of 2027 if ACC issues updated prevention guidance.
